A simple web app for teachers, organizations, or anybody that only needs a form to administer a multiple choice test/survey without the processing, grading, and paper hassle. Fill in the blanks questions are now supported. Answers are automatically processed and graded. This is just an online form so an accompanying test via paper, pdf, docs, or web url will be needed of course. This web app works only in modern html5 browsers such as firefox or chrome.
